Dear Patient,
For many years Derry Medical Center and Londonderry Family Practice has provided you, when necessary, with inpatient care at Parkland Medical Center. However, due to the complexities of providing both outpatient and inpatient care, we have determined that we can no longer do both and instead will focus exclusively on outpatient medicine. Therefore, should you be hospitalized in Derry at Parkland Medical Center, the In-Patient Physicians of Derry at Parkland will provide you with the required care.
We are confident the In-Patient Physicians of Derry will provide exceptional care to our patients, and are taking the necessary steps to ensure as seamless a transition as possible. Appropriate arrangements are being made so that the Inpatient Physicians of Derry can be in communication with your primary care physician and have access to our Electronic Medical Record, so that the information necessary to your care will be readily available to them. Furthermore, your primary care physician will be available for follow up appointments and continuing care as needed.
We anticipate this change will occur in January and want to again emphasize that we have the utmost confidence in the Hospitalists and know you will be in good hands.

Parkland Medical Center Hospitalists
At Parkland Medical Center, we provide continuous care to
patients, made possible by a talented team of hospitalists
who are never more than a phone call away -- day or night.The hospitalists are a group of internal medicine physicians and physician assistants whose speciality is caring for patients in the hospital. They do not provide patient care outside of the hospital -- when you return home, your primary care physician will continue to provide medical care for you. Throughout your stay, your hospitalist will be in contact with your primary care physician. After discharge, written information will be forwarded to your physician's office so that he/she will have a record of your hospital stay in order to insure continuity of your medical care after discharge.
In the hospital, you may be cared for by more than one doctor or physician assistant hospitalist depending on the times of day you are seen and your length of stay. You can be assured that these caregivers will communicate regularly with you and each other to provide seamless care that is responsive to your needs and goals.
