The staff at Diabetes Healthcare of New Hampshire initiate, educate, and establish optimal health care practices for a lifetime of emotional and physical well being. We’ll work with you to help you achieve better health one day at a time.
Pre-Diabetes and Diabetes
Whether you’re borderline or diagnosed with diabetes, we’ll teach you how proper nutrition, exercise, and medication can delay or prevent type 2 diabetes.
Insulin Pump Therapy
We’ll help you determine if an insulin pump is appropriate for you, your condition and your lifestyle. If an insulin pump is recommended, we’ll provide one-on-one insulin pump education including nutrition and exercise. We’ll also monitor and manage insulin pump progression to ensure that you’re on the right track.
Nutrition
For diabetics, proper diet and nutrition can make a significant difference in how you feel and how your diabetes is controlled. We’ll perform an initial nutrition assessment and develop individualized meal plan fitting your unique lifestyle and culture.
Diabetes Education
Each month our team of Diabetes educators offer an informational session on the different aspects of diabetes management, including:
- Insulin pump support groups
- Continuous glucose monitoring system and related topics
- Medication management
- Nutrition
- Proper exercise
Diabetes Support Group - NEW!
Looking for information to better manage your diabetes? Attend one of our free educational sessions focusing on various topics relating to diabetes care. These free sessions are held the fourth Wednesday of each month at 6:30 p.m. in our 6 Tsienneto Road location, and will address issues such as
- diagnostic criteria and monitoring;
- meal planning and exercise;
- medications and up-to-date management tools; and
- problem solving and coping.

Better Diabetes Management
We know that having diabetes can be frustrating and overwhelming sometimes. Our staff is available to recommend an individualized diabetic health plan that includes a complete nutrition assessment, recommended exercise program, adjustment of medication regimens, identification of cardiovascular risk factors and appropriate treatment options; and laboratory and diagnostic testing. We also continuously collaborate with your referring health care provider to ensure the best possible outcomes.
Continuous Glucose Monitoring Systems Program
We have the latest continuous glucose monitors systems that allow us to identify blood glucose excursions and trends to help optimize diabetes control.
Evaluation of Cardiac Risk Factors
Through a comprehensive initial assessment all cardiovascular risk factors will be identified and appropriate action will be taken with the collaboration of the referring health care provider.
Pregnancy & Gestational Diabetes Management
Gestational diabetes is high blood sugar that develops during pregnancy. While eating healthy is an important part of any pregnancy, developing an individualized plan that includes a low-glycemic diet can help to keep the effects of gestational diabetes as minimal as possible. Through the program we’ll help you develop a dietary program, perform close glucose monitoring, and initiate and adjust any necessary medication regimen. In close collaboration with your referring health care provider, we’ll help monitor your condition throughout your pregnancy.
